Dr. Gary Brewton has been awarded top honors (Level 3) for Diabetes Care from the non-profit Health Care Incentive Improvement Institute (HCI3).
The Bridges to Care Diabetes Care Recognition award honors endocrinologists and primary care physicians who deliver high-value diabetes care to adult patients. Clinicians who receive this award have demonstrated that their patient-centered model of care (based on available clinical evidence) incorporates the following:
- Deliver high-quality care from the outset of patient contact
- Understand and consider previous treatment history to help avoid inappropriate treatment
- Comprehensive patient assessment and reassessment
- Patient Education
- Shared Decision making.
In addition, clinicians must demonstrate that the population of patients in their care are maintaining superior control over their diabetes according to these specific clinical measures:
- Blood Pressure (BP) control
- LDL control
- HbA1c control
- Documentation of Ophthalmologic exam
- Documentation of Tobacco status and cessation advice and treatment
- Documentation of Nephropathy assessment
- Documentation of Podiatry exam
- Documented Body Mass Index
There are three levels of Bridges to Excellence Diabetes Care Recognition:
Focuses on a clinician-centric view of measurement, looking at individual metrics summed to produce a composite score, with the inclusion of “minimum” performance requirements for all intermediate outcome control measures, both poor and superior (i.e., BP control, LDL control and HbA1c control). Thresholds have been set to focus on above average performance.
Focuses on a combination of clinician and patient-centric measurements. Level II includes the measurement of individual metrics summed to produce a composite score, with the inclusion of “minimum” performance requirements for all intermediate outcome control measures. Also looks at the defect rate of care delivery across poor control measures on a per patient basis. Thresholds have been set to focus on very good performance.
Focuses on patient-centric view of measurement, looking at the defect rate of care delivery across superior control measures on a per patient basis.
Clinicians must demonstrate that they are using advanced processes and delivering all the right care on a per patient basis. Thresholds have been set to focus on exceptional performance.
